Our client is looking to appoint a compassionate and proactive Patient Care Coordinator to support patients throughout their full healthcare journey. This is a highly patient-focused role, ideal for someone who is passionate about delivering outstanding service while working closely with clinical teams.
The Role
As Patient Care Coordinator, you will guide and support patients from their initial enquiry through treatment and on to final discharge. Acting as a central point of contact, you will be required to identify each patient and refer on/ or book in with the correct consultant based on the patients needs whilst ensuring clear, timely, and empathetic communication between patients and their treating clinicians at every stage.
Key responsibilities
- Identifying individual patient needs, booking and referring patients to the appropriate consultants, and maintaining proactive engagement through aftercare and follow-up.
- Gather feedback and provide ongoing support to help ensure a smooth, positive, and well-coordinated patient experience.
- Manage patient workflows efficiently, coordinating timelines from first contact through to discharge.
- The role may involve chaperoning patients during consultations, enabling you to fully understand treatment plans and confidently discuss next steps, including treatment options and associated costs, in subsequent conversations.
- Strong organisational skills, attention to detail, and the ability to manage multiple patient journeys simultaneously are essential.
About You
You will be empathetic, professional, and confident in explaining medical procedures and treatment plans in clear, accessible language. You are comfortable handling sensitive information and supporting patients who may be anxious or present challenging behaviours. You will be able to work independently while collaborating effectively within a multi-disciplinary team.
